We have now moved to our new premises on Oakwell Drive in Crich (accessed off Coasthill)

If you are visiting the surgery by car, the reception and waiting area are accessed from the upper level car park.  Pedestrian access is via the ground floor where both stairs and a lift is available to take you to the reception and waiting area. 

 

Out of Hours

If you require urgent medical attention when the Surgery is closed, you should dial 111. This will connect you to the NHS 111 service free of charge which is provided by a local organisation: Derbyshire Health United. You will be assessed and directed to the most appropriate point of care for your particular needs.

You can also dial 111 if you require health information.

If you require an appointment at the surgery please contact us during normal opening hours.

Please contact the surgery you normally attend. Urgent appointments will be seen the same day but not always by your usual Doctor

 

Test Results

When requesting results, please ring after 2.00pm to Crich or after 11.00am to the branches, when the Receptionists are less busy.

Repeat Prescriptions

pill_spillCrich patients can request their repeat prescriptions either by ringing 01773 852966 and choosing option 2 (between 10.00 - 14.30), by dropping their repeat prescription request slip in at Reception or by filling in the request form on this website. 

If you are a dispensing patient you can request your repeat prescriptions by ringing either of the Dispensaries during opening hours or by filling in the request form on this website.

Patients in the following categories are automatically exempt from prescription charges: children under 16, under 19 and in full-time education, women and men over 60 and those patients who have exemption certificates, etc. You will be asked by our Dispensers to provide proof of exemption.

PLEASE GIVE 48 HOURS NOTICE.   IF YOU HAVE MORE THAN ONE MEDICATION, PLEASE TRY TO ORDER ALL YOUR ITEMS TOGETHER.

You will be required to see the Doctor for periodic review of your medication and the tear off portion of your prescription will inform you of this.
