We have now moved to our new premises on Oakwell Drive in Crich (accessed off Coasthill)

If you are visiting the surgery by car, the reception and waiting area are accessed from the upper level car park.  Pedestrian access is via the ground floor where both stairs and a lift is available to take you to the reception and waiting area. 

 

Out of Hours

If you require urgent medical attention when the Surgery is closed, you should dial 111. This will connect you to the NHS 111 service free of charge which is provided by a local organisation: Derbyshire Health United. You will be assessed and directed to the most appropriate point of care for your particular needs.

You can also dial 111 if you require health information.

If you require an appointment at the surgery please contact us during normal opening hours.

Please contact the surgery you normally attend. Urgent appointments will be seen the same day but not always by your usual Doctor

 

Test Results

When requesting results, please ring after 2.00pm to Crich or after 11.00am to the branches, when the Receptionists are less busy.

Pre-Registration with the practice

register_nowIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.  PLEASE NOTE  - WE WILL ONLY BE ABLE TO REGISTER YOU VIA THIS METHOD IF YOU ALSO COMPLETE THE ONLINE MEDICAL QUESTIONNAIRE (please see below)

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.  Online medical questionnaire for new patients

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
