Confidentiality

Access To Medical Records

blue_folder_lockIn accordance with the Access to Health Records Act 1990 you are entitled to sight of your medical records. There will be a charge for any photocopies that may be requested. You are also entitled to a copy of any referrals to consultants.

Your Right To Privacy

You have a right to keep your personal health information confidential between you and your doctor. This applies to everyone over the age of 16 years and in certain cases to those under 16. The law does impose a few exceptions to this rule, but apart from those you have a right to know who has access to your medical records. Please ask for a leaflet at Reception.

Suggestions and Complaints

If you would like to make any suggestions or comments to improve our service to you, we would like to hear from you and provide suggestion boxes at all our Surgeries. You may write directly or contact the Practice Manager.

We have an in-house complaints procedure and if you are not happy with any aspect of the service you receive from the Practice, please contact the Practice Manager in the first instance.

The Patient Advice & Liaison Service is also available to offer help and advice. They can be contacted on 0800 783 7279. Leaflets are available in Surgery.

OUT OF HOURS

If you require urgent medical attention when the Surgery is closed, you should dial 111. This will connect you to the NHS 111 service free of charge which is provided by a local organisation: Derbyshire Health United. You will be assessed and directed to the most appropriate point of care for your particular needs.

You can also dial 111 if you require health information.

If you require an appointment at the surgery please contact us during normal opening hours.

Appointments

Please contact the surgery you normally attend. Urgent appointments will be seen the same day but not always by your usual doctor

 

Test Results

When requesting results, please ring after 2.00pm to Crich or after 11.00am to the branches, when the receptionists are less busy.
