Appointments

 

If you ring the surgery and require an urgent appointment your call will be ‘triaged’. This means that the receptionist will take your details, ask you for a brief description of the problem and ensure that a Doctor calls you back.

When the Doctor calls you back he/she will assess your problem and decide upon the best way for you to be treated. This may mean that the Doctor will make a face to face appointment for you to visit the surgery that day or it may be that the problem can be dealt with over the phone and you won’t need to come to the surgery.

Routine face to face appointments can be booked up to 6 weeks in advance. You can request a routine telephone appointment with a Doctor or Nurse Practitioner, you will be given a date in which the Clinician will call you between surgery hours. Patients will be given the option as to a morning or afternoon telephone call

Please note, if you are offered a telephone appointment, clinicians will only attempt to call you three times. If the call has been missed, you will not be offered a further telephone call for that day. Please ensure your contact details are up to date.

We do not offer a walk in service and we do not assess acute minor injuries.

Chaperones

Crich Medical Practice is committed to providing a safe, comfortable environment where patients and staff can be confident that best practice is being followed at all times and the safety of everyone is of paramount importance.

All patients are entitled to have a chaperone present for any consultation, examination or procedure where they feel one is required. The chaperone may be a family member or friend. On occasions you may prefer a formal chaperone to be present i.e. a trained member of staff.

Wherever possible we would ask you to make this request at the time of booking your appointment so that arrangements can be made and your appointment is not delayed in any way. Where this is not possible, we will endeavour to provide a formal chaperone at the time of request.

Your healthcare professional may also require a chaperone to be present for certain consultations in accordance with our Chaperone Policy.

When The Surgery Is Closed

Derbyshire County Primary Care Trust is responsible for the provision of medical services to patients when the Surgery is closed. This service is provided Derbyshire Health United for patients in this area. This Surgery is no longer open on Saturday mornings.

If you require urgent medical attention when the Surgery is closed, you can ring 111 and your call will be automatically transferred to the Out of Hours service

A Receptionist will answer your call (the Receptionist will either be from Derbyshire Health United or from NHS Direct, depending on the time of your call) and will take necessary details from you which will then be passed to a Nurse.

A Nurse will then ring you back. The Nurse will either give telephone advice, arrange for you to speak to a Doctor, ask you to attend the Medical Centre, Scarsdale, Nightgale Close, Newbold Road, Chesterfield or arrange for a Doctor to make a home visit, whichever seems the most appropriate need.

There may be occasions when the nurse advises that a 999 emergency ambulance is necessary.

At weekends you may be requested to attend the designated sessions held at either of the local hospitals at Clay Cross or Whitworth, Matlock. Please DO NOT attend the Medical Centre or either of the Hospitals without having first contacted the Out of Hours Service unless you have had an accident with injury requiring attention at a hospital A&E department in which case you may attend Derby Royal Infirmary or Chesterfield Royal Infirmary directly.
