You may be aware that all practices are now required to provide their patients with a named GP who will have overall responsibility for the care and support that our surgery provides for them. Each patient now has a named GP but this does not prevent you from seeing a GP of your choice.
The practice is now able enabled for the EPS2 service. If you have already nominated a pharmacy your prescription will now be sent electronically to the pharmacy you have chosen. If you haven't yet nominated a pharmacy please contact the pharmacy of your choice who will explain how this is done. Please following the link below for a summary of the benefits of EPS.
Please note Crich PPG are looking for new members to join the group - please see detail on how to join in the Patient Group tab below.
The doctors work in partnership and they all provide general medical care, maternity, contraceptive advice, minor surgery, health promotion and child health surveillance services. We are able to fit IUCD (contraceptive coils) in Practice and this service is performed by Dr Sarah Savage and Dr Yolanda Diez following an initital consultation with her.
Patients with chronic diseases will be reviewed regularly often at the times of their medication review. Please alternate between the doctor and nurse to ensure your benefit from the full care. From 1 April 2004 certain standards have been set by the Government for the care of chronic disease which can only be achieved by seeing the nurse as well as the doctor.
We ask however that you see the same doctor for follow-up of chronic diseases and other problems so that he/she is in a good position to oversee your care.
Our Practice Nurses, Sue Paul and Alison Draffan are responsible for all aspects of health promotion, holiday vaccinations, dressings, routine chronic disease management. We also have Healthcare Assistant's who are responsible for some aspect of patient care ie BP checks and Phlebotomy.
Clinics are held at each of our surgeries.
If you are a housebound patient - a home visit can be arranged for you.
The District Nursing Team run a clinic every Monday, Tuesday, Wednesday, Thursday morning. This service is available to any patient who needs blood taking and appointments can be made by ringing 01773 852966. Where appropriate please obtain a form from your GP before attending the clinic.

We are a registered Yellow Fever Centre and are able to offer this service to travellers, whether or not they are registered with the practice. Full payment is required before the vaccination is given.
If you require any vaccinations relating to foreign travel you need to complete a travel questionnaire online or available from reception. It is important to complete the travel questionnaire and return it to the surgery as early as possible (see below) - at least 6 weeks before you travel. You will then be contacted regarding an appointment to receive the vaccinations. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
There is further information about countries and vaccinations required on the links below
